Radical prostatectomy means complete removal of the tumor tissue along with the affected organ. It is considered the «gold standard» for treating prostate cancer, it stops the oncological process at the early stages.

Unlike alternative focal therapy (cryoablation, radiofrequency ablation, irreversible electroporation and ultrasound therapy), radical prostatectomy does not involve the destruction, but rather it means the removal of the tumor mass from the body.

Approaches for radical prostatectomy

There are open, laparoscopic and robot-assisted approaches. It provides a similar functional and oncological result if it is performed by experienced surgeon.  However, laparoscopic and robot-assisted approaches are characterized by less blood loss, a short period of postoperative recovery, and better suture quality.

Nerve-sparing radical prostatectomy

The main advantage of the nerve-sparing technique is that it helps to avoid erectile dysfunction and urinary incontinence - the likely side effects of radical prostatectomy that a patient can experience for life.

The essence of the method is a preserving the neurovascular bundles feeding the penis, as well as providing relaxation and contraction of the urethral sphincter. This operation is more complicated, it requires the skills of high-precision surgery and is possible with special low impact instruments.

It is performed for a low risk prostate cancer (PSA level is less than 10 - 20 ng/ml, cancer cell differentiation is not more than 6 - 7 Gleason, the tumor is precisely limited by the prostate gland).

Robot-assisted or laparoscopic approaches are generally used. Retropubic approach is less often. All of them have a comparable functional and oncological result.

Sexual function is restored in 4-12 months. Firstly spontaneous, then motivated erections appear. The probability of a favorable outcome in men up to 50 years is more than 90 percent, up to 60 years - 75 percent, up to 70 years - 57 percent, over 70 years - 25 percent.

Surgical steps of radical prostactomy

The operation takes 2 - 3 hours. The doctor removes the prostate, adjacent seminal vesicles and part of the deferent duct. If there is reason to believe the spread of cancer to the pelvic lymph nodes, it is also removed.

After elimination of the tumor lesion, an anastomosis is formed — the ends of the urethra and the bladder neck are stitched with special suture. A catheter is inserted into the urethra for urine outflow while the wound heals (7 - 12 days).

Cost of radical prostatectomy

Treatment is free in the Department of Urology at Sechenov University. Open and laparoscopic prostatectomy are presented through policy of obligatory medical insurance, robot-assisted - according to federal quota.

Sometimes there are some difficulties with obtaining a federal quota. In this case, patients, who want to undergo robotic surgery, are forced to use paid medical services.
